Positron Emission Tomography Use in Head and Neck Cancer

ABSTRACT
Objective: The aim is to evaluate the use of positron emission tomography (PET-CT) in the head and neck cancers (HNC) in line with our clinical experience. Material and Methods: A hundred and thirty-three patients who were diagnosed with HNC in the clinic were included in the study between the years 2007-2012. Cancer groups were identified in 8 titles as nasal and paranasal sinus, nasopharynx, oral cavity, oropharynx, hypopharynx and cervical esophagus, larynx, unknown primary neck and salivary glands. PET-CT findings (SUV-max value), disease-related and demographic data were obtained from chart records, recurrence in patients and metastases were not included in the study group. p <0.05 was considered significant. Results: There were 112 male and 21 female patients whose ages ranged from 21 to 93 (mean ± SD, 61 ± 12.7 years). Larynx, with 69 patients (51.9%), was the largest group. According to the groups, the average SUV-max values were 14±8 in paranasal sinus, 12±3 in nasopharyngeal, 11±5 in oral cavity, 14 ± 6 in oropharynx, 14 ± 7in hypopharynx and cervical esophagus 13 ± 6 in larynx, 16 ± 7 in the neck with the unknown primary and 9 ± 4 in salivary glands.Among the pathologies, the most commonly observed one [n = 113 (85%)] was the squamous cell cancer (SCC). There was no difference among the groups in terms of the distribution of SUV-max values (p = 0760). SCC (n = 113). The distribution of the differentiation groups was 34 in grade 1(30%), 59 in grade 2(52%) and 20 in grade 3(18%), respectively. No difference was found among the differentiation groups relating with the distribution of SUV-max values (p = 0331). The effect of the value of the median SUV-max on sex, differentiation and age was not found in SCC group. (p >0.05). Conclusion: Nowadays, the use of PET-CT in the cancer staging and the follow-up of the response to treatment is getting common. SUV-max values in different areas show no difference in the head and neck. This event –even if their areas are different- was connected to the fact that 85% of the pathologies are common. It was concluded that in the SCC, which is the most common cancer type of head and neck, the change of differentiation does not affect the SUV-max involvement.
ÖZET
Amaç: Pozitron emisyon tomografinin (PET-BT) baş-boyun kanserlerinde (BBK) kullanımını klinik deneyimlerimiz doğrultusunda değerlendirmek. Gereç veYöntemler: 2007-2012 yılları arasında kliniğimizde BBK tanısı konan 133 hasta çalışmaya alındı. Kanser grupları burun ve paranazal sinüs, nazofarinks, oral kavite, orofarinks, hipofarinks ve servikal özofagus, larinks, primeri bilinmeyen boyun ve tükürük bezleri olarak 8 başlıkta belirlendi. PET-BT bulguları (SUV-max değeri), hastalığa ilişkin ve demografik veriler dosya kayıtlarından elde edildi. Çalışmada nüks ve metastaz olgulara yer verilmedi. p<0.05 anlamlı kabul edildi. Bulgular: Yaşları 21 ile 93 arasında değişen (ortalama ± SS, 61±12.7 yıl) 112 erkek, 21 kadın hasta vardı. Larinks 69 hasta (%51.9) ile en büyük grup idi. Gruplara göre ortalama SUV-max değerleri burun ve paranazal sinüste 14±8, nazofarinkste 12±3, oral kavitede 11±5, orofarinkste 14±6, hipofarinks ve servikal özofagusta 14±7, larinkste 13±6, primeri bilinmeyen boyunda 16±7 ve tükürük bezlerinde 9±4 idi. Patolojiler arasında en çok gözleneni [n=113 (%85)] yassı hücreli kanserdi (YHK).Gruplar arasında SUV-max değerlerinin dağılımı açısından fark yoktu (p=0.760). YHK (n=113) diferansiyasyon gruplarının dağılımı grade 1’de 34 (%30), grade 2’de 59 (%52), grade 3’te 20 (%18) olgu idi. Diferansiyasyon grupları arasında SUV-max değerlerinin dağılımı açısından fark bulunmadı (p=0.331).YHK grubunda median SUV-max değerinin değerinin cinsiyet, diferansiyasyon ve yaş üzerine etkisi bulunmadı (p>0.05). Sonuçlar: Günümüzde PET-BT’nin kanser evrelemesi ve tedaviye yanıtın izleminde kullanımı yaygınlaşmaktadır. Değişik alanlardaki SUV-max değerlerinin baş-boyun bölgesinde farklılık göstermediği bulunmuştur. Bu durum, alanları farklı da olsa patolojilerin %85’inin ortak olmasına bağlanmıştır. Baş boyun bölgesinin en sık görülen kanser tipi olanYHK’de diferansiyasyon değişiminin SUV-max tutulumunu etkilemediği sonucuna varılmıştır.
