Our Endoscopic Dacryocystorhinostomy Results in Patients with Epiphora

ABSTRACT
Objective: Tear is secreted by the lacrimal glands located mainly in the superolateral part of the orbita. In addition to that, there are a number of accessory glands distributed along the inner surface of the eyelids. The tear secreted from these glands are transferred into the nose through the lacrimal system. Epiphora is the name given to the watery eye caused by the occlusion of the lacrimal drainage pathway that transfers tear into the nose. The treatment option is surgery in epiphora cases. Dacryocystorhinostomy (DCR) is formation of a new passage between the lacimal sac and the nasal cavity, above the occluded portion of the nasolacrimal canal. The surgical procedure can be performed by two different approaches, external and endoscopic endonasal. This study aimed to investigate long term results and complications of endoscopic DCR for patients with epiphora. Material and Methods: Twenty one patients who had endoscopic DCR between March 2004 and February 2007 were retrospectively analysed. Fifteen (71%) of the patients were females and 6 (29%) were males. The patients were consulted with the ophtalmology clinic preoperatively and the epiphora was shown to be related to the occlusion of the nasolacrimal canal by lacrimal lavage, Jones test or dacriocystography. Bicanalicular silicon intubation tube was applied to all patients and tube was left in place for 8 weeks. Results: Four patients (19%) had intranasal synechia, and one (4.7%) had pyogenic granuloma at the site of inferior punctum postoperatively. Epiphora was completely controlled in 16 patients (76.1%), and partially in 2 (9.5%). There was no change in 3 patients (14.2%). Conclusion: Our success rate of endoscopic DCR was found as 85.6%.
ÖZET
Amaç: Göz yaşı, esas olarak orbitanın superolateralinde yerleşmiş olan lakrimal bez tarafından salgılanır. Ayrıca göz kapağının iç yüzü boyunca dağılmış çok sayıda aksesuar bezler de bulunmaktadır. Bu bezlerden salgılanan göz yaşı, lakrimal sistem yolu ile burun içine taşınır. Epifora; göz yaşını, burun içerisine taşıyan lakrimal drenaj yollarındaki tıkanıklık sonucu ortaya çıkan, göz sulanmasına verilen isimdir. Epiforalı olgularda tedavi seçeneği cerrahidir. Dakriyosistorinostomi (DSR), lakrimal kese ile burun boşluğu arasında nazolakrimal kanal tıkanıklığının üst kısmında yeni bir pasaj oluşturulmasıdır. Cerrahi prosedür, eksternal veya endoskopik endonazal yaklaşım olmak üzere iki ayrı yöntem ile gerçekleştirilir. Çalışmamızda epiforalı olgularda uygulanan endoskopik DSR sonuçları ve komplikasyonların araştırılması amaçlanmıştır. Yöntem ve Gereçler: Mart 2004 - Şubat 2007 tarihleri arasında endoskopik DSR operasyonu uygulanan 21 hasta geriye dönük olarak değerlendirildi. Bunların 15’i kadın (%71), 6’sı (%29) erkekti. Hastalar preoperatif dönemde göz kliniği ile konsülte edildi ve lakrimal lavaj, Jones testi veya dakriyosistografi eşliğinde epiforanın nazolakrimal kanal tıkanıklığına bağlı olduğu gösterildi. Tüm hastalara intraoperatif bikanaliküler silikon intübasyon tüpü uygulandı ve sekiz hafta süre ile silikon tüpler yerinde tutuldu. Bulgular: Dört hastada (%19) intranazal sineşi, bir hastada ise (%4.7) alt punktum lokalizasyonunda piyojenik granülom gelişti. On altı hastada (%76.1) epifora şikayetinde tam düzelme, 2 hastada (%9.5) kısmi düzelme sağlandı. Üç hastada (%14.2) ise hiçbir değişiklik gözlenmedi. Sonuç: Endoskopik DSR ile başarı oranımız %85.6 olarak bulundu.
