Evaluation of Patients with Allergic Rhinitis in Alanya Province

ABSTRACT
Objective: In this study, the characteristics of patients diagnosed as allergic rhinitis in Alanya region are presented. The aim of this study was to update our knowledge of allergic rhinitis and to investigate and reveal regional differences in allergic rhinitis patients. Material and Methods: Patients who were diagnosed as allergic rhinitis by history, physical examination, biochemistry and skin prick tests between September 2018-September 2019 at Baskent University Alanya Research and Application Hospital were included in the study. Results: 166 patients were included in the study. 97 (63.25%) of the patients were female and 69 (36.75%) were male. The mean age was 41.25±15.17 years. The age distribution was between 6-80 years. Pediatric patients accounted for 4.8% of our cases. Perenial allergic rhinitis (PAR) was diagnosed in 83.23% and seasonal allergic rhinitis (MAR) in 16.77%. Serum total immunglobulin E (IgE) levels were 93.80±134.17 in the MAR group and 226.56±380.19 in the PAR group. There was a significant difference in IgE levels between the two groups (p=0.003). The most common presenting complaint was nasal obstruction- respiratory distress (25.1%). In 13.8% of the cases, there was another concomitant allergic disease. Additional diseases were observed more frequently in PAR group compared to MAR group (p<0.05). Skin prick test revealed a single allergen sensitivity of 7.8% (n=13) and multiple allergen sensitivity of 92.2% (n=153). The mean number of positive allergens in PAR patients was 6.07±3.8 and in MAR patients was 3±1.9. The difference between the two groups was statistically significant (p<0.01). The most common positivity in skin prick test was found against house mites Dermatophagoides pteronyssinus (52.8%) and Dermatophagoides farinae (52.1%). Conclusion: With this and similar studies, it will be possible to increase awareness of allergic rhinitis and to determine the effects of geographic regional variables on the course of the disease in allergic rhinitis patients.
ÖZET
Amaç: Alanya Bölgesi’nde alerjik rinit tanısı konulan hastaların özellikleri sunulmuştur. Bu çalışma ile alerjik rinit hastalığı ile ilişkili bilgilerimizin güncellenmesi, alerjik rinit hastalarındaki bölgesel farklılıkların araştırılması ve ortaya konulması amaçlanmıştır. Gereç ve Yöntemler: Eylül 2018-Eylül 2019 tarihleri arasında, Başkent Üniversitesi Alanya Araştırma ve Uygulama Hastanesinde öykü, fizik muayene, biyokimya ve deri prick testleri ile alerjik rinit tanısı konan hastalar çalışmaya dâhil edildi. Bulgular: Çalışmaya 166 hasta dâhil edildi. Hastaların 97 (%58,4)’si kadın, 69 (%41,5)’u erkekti. Ortalama yaş 41,25±15,17 olarak belirlendi. Yaş dağılımı 6-80 yaş arasında idi. Pediatrik hastalar vakalarımızın %4,8’ini oluşturmaktaydı. Hastaların %83,23’üne perenial alerjik rinit (PAR), %16,77’sine mevsimsel alerjik rinit (MAR) tanısı konuldu. Serum total immünglobulin E (IgE) düzeyleri ortalaması MAR tanılı grupta 93,80±134,17 iken, PAR tanılı grupta 226,56±380,19 idi. İki grup arasında IgE düzeyi açısından anlamlı farklılık tespit edildi (p=0,003). En sık başvuru şikâyeti burun tıkanıklığı- solunum sıkıntısı (%25,1) idi. Vakaların %13,8’inde eşlik eden başka bir alerjik hastalık bulunmaktaydı. Ek hastalıkların PAR grubunda, MAR grubuna kıyasla istatistiksel olarak daha sık gözlendiği tespit edildi (p<0,05). Deri prick testinde tek bir alerjen duyarlılığı %7,8 (n=13), çoklu alerjen duyarlılığı %92,2 (n=153) olarak tespit edildi. PAR hastalarında ortalama pozitif alerjen sayısı 6,07±3,8, MAR hastalarında ise 3±1,9 olarak belirlendi. İki grup arasındaki fark istatistiksel olarak anlamlı bulundu (p<0,01). Deri prick testinde en sık pozitiflik ev akarları olan Dermatophagoides pteronyssinus (%52,8) ve Dermatophagoides farinae’ye (%52,1) karşı tespit edildi. Sonuç: Bu ve benzeri çalışmalar ile çok çeşitli uzmanlık dallarında görev yapan hekimlerin alerjik rinit ile ilgili farkındalığını artırmak ve alerjik rinit hastalarında, coğrafi bölgesel değişkenlerin hastalığın seyri üzerine olan etkilerini belirlemek mümkün olabilecektir.
