ABSTRACT
The methods for the diagnosis and treatment of globus pharyngeus varies, since the etiology commonly remains unclear. Psycosomatic disorders may be condemned and hence, this symptom is traditionally named as globus hystericus. According to current clinical trials; it is probable that esophageal dysmotility, hypertonisity of the upper esophageal sphincter, disorders of the tyroid gland and laryngopharyngeal reflux (LPR) may be related to globus pharyngeus. 112 patients with LPR associated symptoms, who had no any other upper airway or gastrointestinal disorder, were selected for this study. 24-hour esophageal pH monitorization was performed in all patients using a doublechannel catheter. The incidence of LPR and reflux parameters (number of reflux, percentage of reflux period) of 40 patients with globus pharyngeus were analyzed and compared with the data obtained from another group of 72 patients with symptoms of upper airway and gastrointestinal irritation other then globus sensation. LPR was present in 50 % of the patients with globus pharyngeus. There were no statistically significant differences of the reflux parameters between the two groups. The results of this study indicate that LPR may be responsible from the globus sensation but, the duration or number of reflux episodes are not sufficient to explain the symptomatic difference between the two groups.
ÖZET
Globus farengeus, Kulak Burun Boğaz hekimlerinin sık karşılaştığı, ancak etyolojisi tam olarak anlaşılamadığı için tanı ve tedavi yaklaşımının değişken olduğu bir semptomdur. Etyolojisinde uzun yıllar psikosomatik bozuklukların rol oynadığı düşünülmüştür. Son yıllarda yapılan çalışmalar, özefageal dismotilite, üst özefageal sfinkter hipertonisitesi, tiroid hastalıkları ve larengofarengeal reflünün (LFR) globus farengeusa neden olabileceğini ortaya çıkarmıştır. Globus farengeusun organik etyolojisinde LFR’nin ne oranda sorumlu olduğuna dair veriler değişkendir. Bu çalışmaya LFR ile ilişkili semptomları olan ve bu semptomlarını açıklayabilecek başka bir patoloji bulunamayan toplam 112 hasta alınmış, hastalara iki kanallı, 24-saatlik özefageal pH monitorizasyonu incelemesi yapılmıştır. Çalışmada globus farengeus yakınması ile başvuran 40 hastada LFR insidansı ve reflü parametreleri incelenmiş, bu hastaların asit reflü atağı sayı ve süre yüzdelerine ait parametreleri globus dışındaki üst solunum ve sindirim sistemi irritasyonu semptomlarıyla başvuran ikinci bir gruptaki 72 hastanın verileri ile karşılaştırılmıştır. Globus farengeusu olan hastaların %50’sinde LFR saptanmıştır. Her iki gruptaki hastaların incelenen reflü parametreleri arasında istatistiksel olarak anlamlı fark bulunmamıştır. Bu sonuçlar, globus farengeus yakınması olan hastaların yarısında LFR’nin bu semptomdan sorumlu olabileceğini; ancak semptom farklılığını yaratan etkenin asit reflü ataklarının süre ve sayısı olmadığını göstermektedir.
