ABSTRACT
Congenital cholesteatoma of the petrous apex (CCPA) is a rare entity. Diagnosis is difficult. They must be treated surgically. In this paper, three cases of CCPA managed in our clinic is presented. All of the cases had progresive facial palsy and sensorineural hearing loss and their tympanic membranes were intact in the physical examination. Patients underwent facial decompression using translabyrinthine transcochlear approach and cholesteatoma was totaly excised. None of the patients had cholesteatoma recurrence, facial functions improved in all patients. The other pathologies that can be seen in the petrous apex and must be remembered for differential diagnosis and surgical approach to those lesions were discussed.
ÖZET
Konjenital petröz apeks kolesteatoması (KPAK) nadir görülen bir hastalıktır. Tanısı güçtür. Cerrahi olarak tedavisi gerekir. Bu yazıda kliniğimizde tedavi edilen 3 KPAK olgusu sunulmuştur. Tüm olgularda progresif fasiyal paralizi ve sensörinöral işitme kaybı izlenmiş, yapılan muayenelerinde intakt kulak zarı ile karşılaşılmıştır. Hastalara translabirentin transkoklear yaklaşımla fasiyal dekompresyon yapılmış ve kolesteatom kitlesi total eksize edilmiştir. Hastaların hiç birinde kolesteatoma rekürrensi izlenmemiş, tüm hastalarda fasiyal sinir fonksiyonlarında ilerleme gerçekleşmiştir. Petröz apeks bölgesinde görülebilen ve ayırıcı tanıda hatırlanması gereken diğer patolojiler ve cerrahi yaklaşım tartışılmıştır.
