The Effects of Needle Aspiration and Drainage with Incision on Length of Hospital Stay in Peritonsillar Abscess Management

ABSTRACT
Objective: Peritonsillar abscess is a frequent clinical condition encountered in the practice of otolaryngology. However, there is no consensus in terms of its best treatment. In the treatment of peritonsillar abscess, abscess is drained by incision or needle aspiration. The patient is hospitalized. This retrospective clinical study was performed to compare the effects of needle aspiration and incision and drainage on hospital stay in peritonsillar abscess. Material and Methods: This study was performed on patients with the diagnosis of peritonsillar abscess, between January 2012 and April 2014. A total of 35 patients were enrolled in this study, and were allocated into two groups as the ones treated with needle aspiration or incision and drainage. Data considering the age, gender, clinical symptoms, type of treatment, hospital stay, and laboratory results were collected. Results: Our study included 19 males and 16 females, with a mean age of 30.7±10.07 years. No significant differences were observed regarding baseline characteristics including age or gender between the needle aspiration and the incision and the drainage groups. Additionally, there were no significant differences for the median hospital stay between the two groups. No significant complications were observed in either group. Conclusion: In the treatment of peritonsillar abscess, primarily the abscess should be drained. The drainage method depends on the preference of the surgeon. Needle aspiration is an efficient and safe procedure to treat peritonsillar abscess. As a result, it is suggested that needle aspiration can be performed as the first-line treatment of peritonsillar abscess because it is easier to perform, and it can be easily repeated. The incision method provides the same success rate, and is not disadvantageous in terms of length of hospital stay.
ÖZET
Amaç: Peritonsiller apse, kulak burun boğaz pratiğinde sık rastlanılan bir klinik durumdur. Ancak tedavi açısından henüz bir fikir birliği yoktur. Peritonsiller apse tedavisinde abse insizyon veya iğne ile aspirasyon yapılarak drene edilir. Hasta hastaneye yatırılır. Bu retrospektif klinik çalışma, peritonsiller apsede insizyon ve iğne aspirasyonu ile drenajın hastanede yatış süresine etkilerini karşılaştırmak amacıyla yapılmıştır. Gereç ve Yöntemler: Ocak 2012-Nisan 2014 tarihleri arasında peritonsiller apse tanısı alan olgular retrospektif olarak değerlendirildi. İğne aspirasyonu ve insizyon ile drenaj uygulanan 35 olgu çalışmaya dahil edildi ve iki gruba ayrıldı. Yaş, cinsiyet, klinik belirtiler, tedavi şekli, hastanede kalış süresi ve laboratuar sonuçları gibi bilgiler toplandı. Bulgular: Çalışmamız, yaş ortalamaları 30,7±10,07 olan, 19 erkek ve 16 kadın olguyu içeriyordu. İğne ile aspirasyon ve insizyon ile drenaj grupları arasında yaş ve cinsiyet de dahil olmak üzere temel özellikleriyle ilgili olarak anlamlı bir fark gözlenmedi. Ayrıca iki grup arasında ortalama hastanede kalış süreleri arasında anlamlı farklılık bulunmadı. İki grupta da önemli bir komplikasyon gözlenmedi. Sonuç: Peritonsiller apse tedavisinde öncelikle apse drene edilmelidir. Drenaj için kullanılacak yöntem cerrahın tercihine bağlıdır. İğne ile aspirasyon, peritonsiller apse tedavisinde etkili ve güvenli bir yöntemdir. Sonuç olarak, daha kolay uygulanması, kolaylıkla tekrarlanabilmesi, insizyon yöntemi ile aynı başarıyı sağlaması ve hastanede kalış süresi bakımından bir dezavantaj oluşturmaması nedeniyle peritonsiller apse tedavisinde ilk basamak tedavi olarak iğne ile aspirasyonu önermekteyiz.
