EFFECTIVENESS OF MODIFIED EPLEY MANEUVER IN THE TREATMENT OF PATIENTS WITH POSTERIOR CANAL BENIGN PAROXYSMAL POSITIONAL VERTIGO

ABSTRACT
In this prospective study, a modification of Epley’s maneuver was used in the treatment of 25 patients (15 female, 10 male, average age: 38.7) with posterior canal benign paroxysmal positional vertigo (p-BPPV). Patient history and the obsevation of the typical nistagmus on Dix-Hallpike maneuver were the diagnostic criteria for p-BPPV. Patients were asked to score how their disorder affected their lives. Then modified Epley maneuver was performed and the patients were supplied with the instructions that they had to obey the following 10 days. On the 10th day and after 1 month, patients were again asked to score their quality of life. Additionally to the scoring at the end of the first month; Dix-Hallpike maneuver was performed to see if nistagmus existed. Only one patient needed a second reposition manever to be releaved of symptoms. Complete resolution of sypmtoms and absence of nystagmus were the criterion for cure. At the end of first month, 96% of patients were successfully cured. On follow-up (average follow-up time: 9 months), only one patient suffered recurrence. Yet the quality of life was clearly improved. Modified Epley maneuver provides effective treatment in patients with p-BPPV.
ÖZET
Posterior semisirküler kanal orijinli benign paroksismal pozisyonel vertigolu (p-BPPV) 25 hastanın (15 kadın, 10 erkek; yaÅŸ ortalama-sı:38.7) dahil edildiÄŸi bu prospektif çalışmada modifiye Epley manevrasının tedavideki etkinliÄŸi deÄŸerlendirildi. Hastalara tanı, öykü ve Dix-Hallpike testinde tipik özellikleri olan nistagmusun görülmesiyle konuldu. Hastalardan ÅŸikayetlerinin yaÅŸamlarına olan etkilerini skorIamalan istendi. Hastalara tam konulduÄŸu anda modifiye Epley manevrası uygulandı ve 10 gün boyunca uyulması gereken talimatlar verildi. Manevra uygulamasını takip eden 10. gündeki ilk ve 1. aydaki ikinci kontrolde hastaların yaÅŸam kaliteleri tekrar skorlandı. Ek olarak ikinci kontrolde Dix-Hallpike manevrası ile nistagmus araÅŸtırıldı. Sadece 1 hastada ikinci manevra uygulamasına ihtiyaç duyuldu. Åžikayetlerin tamamen ortadan kalkması ve provakatif manevrada nistagmusun bulunmaması tam baÅŸarı olarak tanımlandı ve ikinci kontrolde %96 oranında tam baÅŸarı elde edildi. Ortalama olarak 9 ay takip edilen hastalardan birinde rekürrens gözlendi.Tedavi sonrasında hastaların ya-ÅŸam kalitelerinde ise istatistiksel olarak anlamlı bir ÅŸekilde iyileÅŸme saptandı. Sonuç olarak modifiye Epley manevrası p-BPPV’lu hastaların tedavisinde yüksek oranda baÅŸarı saÄŸlamaktadır.
