A TECHNIQUE FOR THE RECONSTRUCTION OF THE POSTERIOR MEATAL WALL AND MASTOID OBLITERATION IN RADICAL CAVITY SURGERY

ABSTRACT
In large radical mastoidectomy cavities, discharging granulations and retention of the debris in the cavity of ten lead to chronic and refractory infections and hearing impairment. To reduce the size of the radical cavity and restore the middle ear functions, we employed a one-stage procedure with ossiculoplasty, reconstruction of the posterior meatal wall and obliteration of the mastoid cavity with autograft bone chips in five patients between 1993 and 1994. The patients were followed up for a period between 6 months and l year, in the postoperative period, the discharge stopped immediately, no debris retention was present in the external ear canal, a middle ear space containing air and a vibrating ear drum was established. The results suggest that mastoid obliteration technique using autograft bone is an effective method for restoration of the functional anatomy of the middle ear and external ear in radical cavity surgery. This paper deals mainly with the surgical technique and its structural and functional implications.
ÖZET
Radikal mastoidektomili hastalarda kavitede oluşan granülasyonlara ve defans birikmesine bağlı olarak kronik ve tedaviye dirençli enfeksiyonlar ve işitme kaybı gelişmektedir. Radikal kavitenin küçültülmesi ve orta kulak fonksiyonlarının restorasyonu amacı ile 1993 - 1994 arasında 5 hastada tek kademeli operasyonda radikal kavite revizyonu, ossiküloplasti, otogreft kemik plakalarla dış kulak yolu arka duvar rekonstrüksiyonu ve mastoid obliterasyon uyguladık. Hastalar 6 ay ile l yü arasında takip edildi Postoperatif dönemde hastalarda akıntının hemen kaybolduğu, dış kulak yolunda debris birikmediği, havalı bir orta kulak ve hareketli bir zar bulunduğu görüldü. Bu sonuçlar radikal kavite cerrahisinde otogreft kemiklerle mastoid obliterasyon tekniğinin orta kulak ve dış kulak yolunun fonksiyonel anatomisinin restorasyonu için efektif bir yöntem olduğunu göstermektedir. Yazıda cerrahi teknik tarif edilmiş ve tekniğin anatomik ve fonksiyonel özellikleri tartışılmıştır.
