Results of Endoscopic Sinus Surgery in Asthmatic Patients with Chronic Inflammatory Sinonasal Disease

ABSTRACT
Asthma is a chronic inflammatory disease of lower airways. The relationship between asthma and chronic inflammatory sinonasal disease (CISND) is still debated and various mechanisms that explain this relationship are proposed. But it’s well known that there exits CISND in the most of the patients with asthma. In this article, the results of the endoscopic sinus surgery (ESS) in 30 patients with non-allergic nonpolyposis CISND (chronic sinusitis) and asthma between 1996-1999 were evaluated. Ten percentage (3/30), 16.7% (5/30) and 73.3% (22/30) of the patients were severe, moderate and mild persistent asthma, respectively. After the ESS asthma symptoms and findings were improved in 80% (24/30) of the patients and these patients were accepted successful. As a conclusion, medical treatment is essential in asthmatic patients and ESS is the complementary part of the treatment and the best surgical method in patients with asthma and CISND. In addition, ESS is also the important to prevent the triggering effect of CISND.
ÖZET
Astım, hava yollarının kronik inflamatuar bir hastalığıdır. Kronik inflamatuar sinonazal hastalıklar (KİSNH)’la iliskisi halen tartısmalıdır ve iliskiyi açıklayan değisik mekanizmalar ileri sürülmüstür. Fakat bilinen bir gerçek astımlı olguların büyük çoğunluğunda KİSNH’ın varlığıdır. Bu çalısmada 1996-1999 yılları arasında astım tanısı alan nonallerjik ve polipsiz KİSNH’lı (kronik rinosinüzitli) 30 olguda, endoskopik sinüs cerrahisi (ESC) sonuçları değerlendirilmistir. Olguların %10’u (3/30) ağır persistan, %16.7’si (5/30) orta persistan ve 73.3’ü (22/30) hafif persistan astımlı idi. ESC sonrası olguların %80’inde (24/30) astım semptom ve bulgularında düzelme tespit edilmis ve basarılı kabul edilmistir. Astımlı olgularda primer tedavinin medikal olduğu, astımın KİSNH’la birlikteliğinde ESC’nin tamamlayıcı ve uygulanabilecek en iyi cerrahi tedavi olduğu inancındayız. Ayrıca, KİSNH’ın tetikleyici etkisinin ortadan kaldırılmasında önemli rol oynadığını da vurgulamak isteriz.
