Stimulation Thresholds and Response Parameters of the Facial Nerve as Prognostic Factors in Facial Nerve Exploration

ABSTRACT
Objective: To assess the stimulation thresholds, latencies and amplitudes of the response of the facial nerve as the prognostic factors in surgical procedures where the facial nerve is at risk. Material and Methods: Intraoperative facial nerve monitorization was applied to 31 patients (23 males, 8 females) between May 2005 and March 2007. Facial function was graded according to the House Brackmann Grading system, and the average follow up period was 9.9 ± 5.5 months. Results: There was no statistically significant difference in stimulation thresholds between postoperative paralytic (seven cases), and non paralytic cases (19 cases); however, significant differences were noted in comparison of latency and amplitude values of the response between paralytic and non paralytic cases. Latency values of the paralytic group were usually above 5 msc (5.28 ± 0.54 msc and 5.32 ± 0.50 msc ). Amplitude values were below 0.2 V (0.15 ± 0.06 V and 0.16 ± 0.09 V). Latency values in the non paralytic grup were below 3 msc (3.00 ± 0.50 msc and 2.93 ± 0.41 msc) and the amplitude values were above 0.5 V (0.54 ± 0.22 V and 0.55 ± 0.29 V). Conclusion: In prediçtion of postoperative prognosis, stimulation threshold was not found as a reliable parameter. In contrast, the results showed that latency and amplitude values of the response might be used as reliable parameters for estimation of postoperative prognosis.
ÖZET
Amaç: Fasiyal sinirin risk altında olduğu prosedürlerde; intraoperatif uyarılma eşikleri, latans ve amplitüd ölçümlerinin, postoperatif prognoza etkisinin değerlendirilmesi amaçlanmıştır. Yöntem ve Gereçler: Mayıs 2005-Mart 2007 tarihleri arasında toplam 31 hastaya (23 erkek, 8 kadın) intraoperatif fasiyal sinir monitörizasyonu uygulanmıştır. Hastaların fasiyal fonksiyonları, House Brackmann evreleme sistemi ile değerlendirilerek operasyon sonrasında ortalama 9.9 ± 5.5 ay takip edildi. Bulgular: Ameliyat sonrasında fasiyal paralizi gelişen yedi olgu ile paralizi gelişmeyen 19 olgu arasında uyarılma eşikleri açısından anlamlı fark bulunmazken, latans ve amplitüd değerleri açısından istatiksel olarak anlamlı fark bulundu. Paralitik olan grubun latans değerleri genelde 5 msn’nin üzerinde ( ort. 5.28 ± 0.54 msn ve 5.32 ± 0.50 msn), amplitüd değerleri ise 0.2 V’un altında bulunmuştur (ort. 0.15 ± 0.06 V ve 0.16 ± 0.09 V). Nonparalitik grupta ise latans değeri 3 msn’nin altında ( ort. 3.00 ± 0.50 msn ve 2.93 ± 0.41 msn), amplitüd değeri ise 0.5 V’un üzerinde bulunmuştur (ort. 0.54 ± 0.22 V ve 0.55 ± 0.29 V). Sonuç: Uyarılma eşiğinin ameliyat sonrası prognoz tayininde çok güvenilir bir parametre olmadığı, latans ve amplitüd değerlerinin ise güvenilir parametreler olduğu söylenebilir. Latans değerinin 5 msn üzerinde olması, amplitüd değerinin ise 0.2 V’un altında olması fasiyal paralizi için risk teşkil edebilir.
