THE CLINICAL IMPORTANCE OF VIDEOLARYNGOSTROBOSCOPY IN THE DIAGNOSIS AND MANAGEMENT OF VOICE DISORDERS

ABSTRACT
Videolaryngostroboscopy (VLS) is the most valuable visualization method in the evaluation of the larynx. The use of VLS which allows a better assesment of vocal fold vibrations and morphology of larynx has been widely accepted in the diagnosis and management of laryngeal disorders. VLS gives detailed information about the diagnosis, management and follow up of the laryngeal pathologies, voice disorders and conditions in which indirect laryngoscopy is not helpful. This diagnostic tool has many advantages as it is easy to use and magnifies the image which can be recorded on a videotape. A pre-and post management comparison of the pathologies can be easily done by VLS and it can be used as a training tool far medical purposes. In this series, 3070 VLS procedures were performed on 1946 patients. Organic pathologies were diagnosed in 1433 patients while functional pathologies were in 250 patients. No pathology was detected in 15.7% of the cases (306/1946) with voice complaint. A number of varied pathologies were found simultaneously in the same patient while the follow up course demonstrated different pathologies in the same patients. The most commoly seen pathologies were polyps (222/1946), reinke edema (162/1946) (8.3%) and carcinomas (141/1946) (7.2)%. VLS is an invaluable investigation method in the early detection of organic pathologies, particularly small carcinomas and precancerous lesions, reflux laryngitis and early phases of varicous formations. in this series, the lesions which were mostly diagnosed as bilateral nodules by indirect laryngoscopy were in fact vocal fold cysts with a contralateral reactive mass in the contralateral vocal fold. VLS played a very important role in the diagnosis and management of functional dysphonias. It is used as a visual feedback tool in the management of muscle tension dysphonias (MTD) particularly in mutational falsetto(MTD6) and conversion aphonia (MTD 4).
ÖZET
Videolarengostroboskopi (VLS); günümüzde larenksin incelenmesinde kullanılan en geçerli görüntüleme yöntemidir. Larenksin morfolojik yapısı ve vokal foldlarm hareket özelliklerinin daha iyi değerlendirilmesini sağlayan VLS\'nin larenks hastalıklarının tanı ve tedavisinde kullanımı giderek yaygınlaşmaktadır. İndirekt larengoskopi ile saptanamayan larengeal patolojilerin tanısında VLS değerli bilgiler verir. Kullanım kolaylığı, net ve büyütülmüş görüntü sağlaması, tedavi öncesi ve sonrası karşılaştırmaya olanak vermesi, video ortamına kayıt yapılabilmesi hastaya hastalığı hakkında görsel bilgi verilmesinin sağlanması ve tıp eğitiminde kullanılabilmesi nedeniyle klinik önemi çok fazla olan bir tanı aracıdır. Bu seride 1946 hastada 3070 VLS işlemi yapıldı. 1433 hastada organik patoloji, 250 hastada fonksiyonel patoloji tespit edildi. Hastaların %15.7\'sinde ses şikayeti olmakla birlikte VLS\'de bir patoloji tespit edilemedi. Aynı hastada birden fazla patoloji bir arada bulanabildiği gibi, aynı hastada değişik zamanlarda değişik patolojiler de bulundu. En sık görülen patolojiler; polip (222/1946) (%11.4), larenjit (212/1946) (%10.8), nodul (170/1946) (%8.7), reinke ödemi (162/1946) (%8.3) ve kanser (141/1946) (%7.2) idi. VLS; organik lezyonlar içinde özellikle küçük kanserler, prekanseröz lezyonlar, reflü larenjit ve variköz oluşumların erken dönemde tespitinde çok yararlı oldu. Çoğu kere bilateral nodul olarak değerlendirilen lezyonların aslında karşı vokal foldda reaktif kitle ile birlikte seyreden vokal fold kistleri olduğu tespit edildi. VLS fonksiyonel disfonilerin tanısı ve tedavisinde de çok önemli rol oynadı. Özellikle mutasyonel falsetto (KGD6) ve konversiyon afoni (KGD 4) olmak üzere tedavide vizüel bir geribildirim, aracı olarak kullanıldı.
