Comparison of Cartilage Island Graft with Perichondrium and Temporalis Muscle Fascia in Tympanoplasty Type 1

ABSTRACT
Objective: To evaluate the surgical results in terms of graft success and hearing gain in cases who underwent type 1 tympanoplasty using fascia or cartilage as graft material. Material and Methods: Among the cases who had type 1 tympanoplasty for chronic otitis media between October, 2008 and December, 2009 26 patients with cartilage island graft and 61 cases with temporal fascia were included in the study. Tragal cartilage and conchal cartilage from cymba were used in cases where cartilage island graft was used. Graft success at postoperative months 1, 3, 6 and hearing gain at the end of sixth month were evaluated. Pure tone average, airway hearing thresholds at 500 Hz, 1000 Hz, 2000 Hz ve 4000 Hz, and average air-bone gap were used as parameters to evaluate hearing gain. Results: As the postoperative follow up period increased, graft success was found to be significantly decreased in cases with fascia compared to patients with cartilage graft (p< 0.05). Graft success rate with cartilage graft (%89.5) was higher than that of fascia graft (p< 0.05). When the hearing gain was evaluated, significant improvement of hearing levels compared to preoperative period was observed in both groups. Hearing gain ( preoperative-postoperative mean air-bone treshold difference) in fascia group and cartilage group were 7.28 ± 9.31 dB and 9.18 ± 9.55 dB, respectively. Conclusion: Cartilage graft usage in type 1 tympanoplasty is more advantageous compared to fascia in terms of graft success, and this method has satisfactory hearing gain results.
ÖZET
Amaç: Tip 1 timpanoplastide greft materyali olarak fasya veya kıkırdak kullanılan olgularda cerrahi sonuçların greft baÅŸarısı ve iÅŸitme kazancı açısından deÄŸerlendirilmesi. Gereç ve Yöntemler: Ekim 2008-Aralık 2009 tarihleri arasında kronik otitis media nedeniyle tip 1 timpanoplasti yapılan olgulardan kıkırdak ada grefti kullanılan 26 olgu ve temporal fasya kullanılan 61 olgu çalışmaya dahil edildi. Kıkırdak ada grefti kullanılan olgularda tragal kıkırdak ve simbadan alınan konkal kıkırdak kullanıldı. Hastaların postoperatif 1. ay, 3.ay ve 6. aydaki greft baÅŸarıları ve 6. ayın sonundaki iÅŸitme kazançları deÄŸerlendirildi. Ä°ÅŸitme kazançlarının deÄŸerlendirilmesinde saf ses ortalaması (SSO), 500 Hz, 1000 Hz, 2000Hz ve 4000 Hz’deki hava yolu iÅŸitme eÅŸikleri ve ortalama hava-kemik yolu aralığı ölçüt alındı. Bulgular: Greft materyali olarak fasya kullanılan olgularda kıkırdak kullanılan gruba oranla postoperatif takip süresi arttıkça greft baÅŸarısında anlamlı düşme tespit edildi (p< 0.05). Kıkırdak greftle elde edilen greft baÅŸarısı (%89.5), fasyaya oranla daha yüksekti (p< 0.05). Ä°ÅŸitme kazancı deÄŸerlendirildiÄŸinde, her iki hasta grubunda da preoperatif döneme göre iÅŸitme seviyelerinde anlamlı düzelme tespit edildi. Fasya grubunda iÅŸitme kazancı (preoperatif-postoperatif ortalama hava-kemik yolu eÅŸiÄŸi farkı) 7.28 ± 9.31 dB iken kıkırdak grubunda 9.18 ± 9.55 dB idi (p> 0.05). Sonuç: Tip 1 timpanoplastilerde kıkırdak greft uygulaması greft baÅŸarısı açısından fasya kullanımına göre daha avantajlı, iÅŸitme kazancı açısından da tatmin edici sonuçlara ulaşılabilen bir yöntemdir.
