Bony Destruction in Chronic Otitis Media with or without Cholesteatoma

ABSTRACT
Objective: Clinically, chronic otitis media can be divided into two groups as chronic otitis media with or without cholesteatoma. The major and minor complications related to this pathology is due to the bony destruction. Thus, the bony structure destruction in chronic otitis media with or without cholesteatoma were evaluated. Material and Methods: 280 patients that were operated between the years 2000 and 2005 in our clinic were included in this study. 99 patients who were operated previously once again, who had type 1 tympanoplasty, and who had had facial nerve decompression were excluded. The remaining 181 patients were evaulated and ossicular destruction, facial nerve dehissances and other bony defects were noted. Result: In chronic otitis media with cholesteatoma , ossicle destruction was found as 84%. In chronic otitis media without cholesteatoma, ossicular destruction was found as 42%. Facial nerve dehissance was found 35% in the group of patients with cholesteatoma and 3% in the group of patients without cholesteatoma. Lateral semicircular canal and tegmen defects were detected in 10% in chronic otitis media with cholesteatoma and 1.2% in chronic otitis media without cholesteatoma. Conclusion: According to these data, a large number of bone defect was defined in the surgery of chronic otitis media with or without cholesteatoma. Knowing that these kinds of defects can be seen, provides the surgeon enter the operation more conscious and prevents the probable complications.
ÖZET
Amaç: Kronik otitis media klinik olarak olarak kolesteatomlu ve kolesteatomsuz kronik otitis media olarak ikiye ayrılabilir. Bu patolojiye bağlı oluşan minör veya majör komplikasyonların büyük bir kısmı kemik yapı hasarlarına bağlıdır. Buna bağlı olarak kolesteatomlu ve kolesteatomsuz kronik otitis mediada görülen kemik yapı hasarları değerlendirildi. Yöntem ve Gereçler: Kliniğimizde kronik otititis media nedeni ile 2000- 2005 tarihleri arasında opere olan ve dosyasına ulaşılan 280 hasta değelendirmeye alındı. Bu hastalardan daha önce opere olan, tip1 timpanoplasti veya fasiyal sinir dekompresyonu yapılan 99 hasta çalışmaya alınmadı. Geri kalan 181 hasta retrospektif olarak değerlendirilerek kemikçik zincir, kemik yapı hasarları ve fasiyal kanal açıklığı görülme oranları belirlendi. Bulgular: Kolesteatomlu kronik otitis media’da kemikçik zincir hasarı %84 oranında bulundu. Kolesteatomsuz kronik otitis media’da ise kemikçik zincir hasarı %42 olarak görüldü. Fasiyal kanal açıklığı ise kolesteatomlu kronik otitis media’lı hastaların %35’de bulunurken, kolesteatomsuz kronik otitis medialı hastaların % 3’ünde bulundu. Lateral semisirküler kanal ve tegmen antri kemik hasarı, kolesteatomlu KOM’lı vakaların % 10’nunda, kolesteatomsuz vakaların %1.2’sinde saptanmıştır. Sonuç: Elde edilen veriler ışığında, kolesteatomalı ve kolesteatomasız kronik otitis medianın cerrahi tedavisi sırasında büyük sayıda kemik yapı hasarının varlığı tespit edildi. Bu kemik hasarlarının önceden var olabileceğinin bilinmesinin operasyona cerrahın daha bilinçli girmesi ve operasyon sırasında ortaya çıkabilecek komplikasyonların önlenmesi açısından büyük önem taşımaktadır.
