Brown Tumor as a Cause of Mass in the Oral Cavity

ABSTRACT
Brown tumors (osteitis fibrosa cystica) are non-neoplastic lesions caused by abnormal bone metabolism due to hyperparathyroidism. It is often accompanied by secondary hyperparathyroidism, it is more rare to accompany primary hyperparathyroidism. The Brown tumor is often seen in the ribs, clavicle and pelvis, but may involve any bone in the skeletal system, including maxillofacial bones. The mandible is held more frequently than the maxilla in the chin area. They are clinically smooth, soft and bone destructive lesions. These lesions may mimic malignancy clinically and radiologically. While there is a common view of the intervention of the parathyroid glands that cause Brown tumor treatment, there is no consensus regarding the intervention for Brown tumor. In this article, four Brown tumor cases secondary to primary hyperparathyroidism that cause mass lesion in the oral cavity and our approach to cases are discussed in the light of the literature.
ÖZET
Brown tümör (osteitis fibroza sistika), hiperparatiroidiye bağlı anormal kemik metabolizması sonucu oluşan nonneoplastik lezyonlardır. Sıklıkla sekonder hiperparatiroidiye eşlik etmektedir, primer hiperparatiroidili hastalarda daha nadir görülmektedir. Brown tümör sıklıkla kostalar, klavikula ve pelviste görülmekle birlikte maksillofasiyal kemikler dâhil iskelet sistemindeki her kemiği tutabilir. Çene bölgesinde mandibula maksilladan daha sık tutulmaktadır. Klinik olarak düzgün yüzeyli, yumuşak ve kemik destrüksiyonu yapan lezyonlardır. Bu lezyonlar, klinik ve radyolojik olarak maligniteyi taklit edebilir. Brown tümör tedavisinde hastalığa sebep olan paratiroid bezlerine yönelik cerrahi müdahale konusunda ortak görüş varken, Brown tümöre yönelik müdahale konusunda fikir birliği yoktur. Bu yazıda, oral kavitede kitle lezyonu yapan primer hiperparatiroidiye sekonder gelişen 4 Brown tümörlü olgu ve olgulara yaklaşımımız literatür eşliğinde tartışılmıştır.
