Comparison of Fine Needle Aspiration Biopsy and Histopathological Findings in Salivary Gland Masses

ABSTRACT
Objective: The aim of this study is to determine the sensitive, specifity and diagnostic value of fine needle aspiration biopsy (FNAB) and in salivary gland masses. Material and Methods: Patients operated for parotis and submandibular gland masses between 2014-2019 were included in the study. Demographic data, preoperative FNAB and postoperative patholgical examination results of the patients were recorded. Patients divided into two groups as parotid group (PG) and submandibular gland group (SG). Spesivity, sensitivity, positive predictive value and negative predictive values were evaluated to investigate the relationship between preoperative FNAB and postoperative definitive pathology results. Results: PG had 0.9% false positivity rate while false negativity rate was 50%. Among PG; FNAB had 33.3% sensitivity and 98.9% specificity, 80% positive predictive value and 95.8% negative predictive value. Accuracy rate was 95% in parotis masses. SG had 66.7% false negativity and 0% false positivity. Among SG; FNAB had 33.3% sensitivity and 100% specificicty. Accuracy rate was 85.7%. Conclusion: If FNAB reveals a malignant mass; this mass should not be directly accepted as malignant. Submandibular gland FNAB has higher specifity rates than parotis FNAB. Although FNAB yield false positive or false negative results; it might help to clinican during preoperative period.
ÖZET
Amaç: Tükürük bezi kitlelerinde ince iğne aspirasyon biyosisi (İİAB)’nin tanısal değerini, duyarlılık ve özgüllüğünü ve tanısal değerini belirlemektir. Gereç ve Yöntemler: Çalışmaya, 2014-2019 yılları arasında parotis ve submandibular bez kitlesi nedeni ile ameliyat edilen hastalar dâhil edildi. Hastaların demografik verileri, preoperatif İİAB sonuçları ve postoperatif patolojik tanıları kaydedildi. Hastalar parotis grubu (PG) ve submandibular bez grubu (SG) olarak 2’ye ayrıldı. İİAB ve nihai patoloji arasındaki ilişkiyi araştırmak için spesifisite, sensitivite, olumlu öngörü ve olumsuz öngörü değerleri hesaplandı. Bulgular: PG’de yanlış pozitiflik %0,9, yanlış negatiflik %50 idi. PG’de İİAB’nin duyarlılığı %50 ve özgüllüğü %98,9; olumlu öngörü değeri %80 iken, olumsuz öngörü değeri %95,8, toplam doğruluk oranı %95 olarak saptandı. SG’de yanlış negatiflik %66,7 iken, yanlış pozitiflik %0 olarak bulundu. SG’de İİAB’nin duyarlılığı %33,3 ve özgüllüğü %100, olumlu öngörü değeri %100 iken, olumsuz öngörü değeri %95,8 idi. Toplam doğruluk oranı %85,7 olarak saptandı. Sonuç: Parotis bezinde İİAB eğer malign bir kitleyi işaret ediyor ise kitle doğrudan malign olarak kabul edilmemelidir. Submandibular bezde İİAB özgüllük oranı daha yüksek bulunmuştur. İİAB; yanlış negatif ve yanlış pozitif sonuçlar vermesine rağmen tedavi planlanmasında klinisyene yardımcı olabilir.
