Pulsatile tinnitus is an uncommon symptom produced by a variety of causes. Most common presenting symptom of dural arteriovenous fistulas (AVF) is pulsatil tinnitus and these fistulas may also cause intracranial hemorrhage. In this paper we describe the case of a patient who had pulsatile tinnitus that was caused by dural AVF. In this case, embolisation of dural AVF was done with endovascular coil and polyvinyl alcohol (PVA) particles and complete resolution of tinnitus was observed. There was no complication. She stayed one day in the hospital after embolisation. On the basis of this demonstrative case we discussed the clinical presentation, pathogenesis, diagnosis, principles of treatment of dural AVFs and proprerties of embolisation treatment.

Pulsatil tinnitus çesitli nedenlerden dolayı ortaya çıkabilen ve sık karsılasılmayan bir semptomdur. Dural arteriovenöz fistülle- rin (AVF) en sık izlenen semptomu pulsatil tinnitus olup, bu fistüller intrakranial hemorajilere sebep olabilmektedir. Bu makalede pulsatil tinnitus nedeni olarak dural AVF tesbit edilen bir olgu sunulmustur. Pulsatil tinnitusta sıklıkla teshis asamasında problemler yasanabilmektedir. Doppler ultrasonografi ve selektif arteriografi nedene yönelik temel bilgileri sağlamaktadır. Sunulan olguda selektif endovasküler platin coil ve polivinil alkol (PVA) partikülleri kullanılarak dural AVF oklüde edilmis olup tinnitusun tamamen ortadan kalkması sağlanmıstır. İslemden sonra komplikasyon gelismemis ve hasta bir gün takip edildikten sonra taburcu edilmistir. Bu demonstratif olgu nedeni ile dural AVF’ün klinik bulguları, patogenezi, teshis için kullanılan görüntüleme yöntemleri, tedavi prensipleri ve embolizasyon tedavisinin özellikleri tartısılmıstır.
