Hereditary hemorrhagic telangiectasia is an otosomal dominant angiogenesis disorder characterized by presence of telangiecta- sia in mucocutaneous tissue, solid organs and central nervous system. Uncontrollable epistaxis despite therapy is frequent. Na- sal packing, electrocauterization, medical cauterization, hormone therapy and antifibrinolytic agents may be used for the treatment of epistaxis. Septal dermoplasty is an alternative method for the patients that did not benefit from those measures. In this report, a case with hereditary hemorrhagic telangiectasia is presented to whom we performed endoscopic septal dermoplasy and controlled his epistaxis. Septal dermoplasty, usually combined with inferior turbinectomy, is performed while preserving the anatomical integrity of the inferior turbinate.

Herediter hemorajik telanjiektazi mukokutanöz dokularda, iç organlarda ve santral sinir sisteminde vasküler telanjiektazilerin varlığıyla karakterize, nadir görülen bir otozomal dominant anjiogenesis bozukluğudur. Genellikle, tedaviye rağmen kontrol altına alınamayan burun kanamaları görülür. Burun kanamaları için tampon uygulaması, elektrokoterizasyon, medikal koterizasyon, hormon tedavisi ve antifibrinolitik ajanlar gibi tedaviler kullanılabilir. Bu tedavilerden fayda görmeyen hastalarda septal dermoplasti tekniği alternatif bir yöntemdir. Bu makalede, endoskopik septal dermoplasti uyguladığımız ve burun kanamalarını kontrol altına aldığımız herediter hemorajik telanjiektazili bir olgu sunulmustur. Genellikle inferior konka rezeksiyonu ile kombine edilen septal dermoplasti, bizim olgumuzda alt konkanın anatomik bütünlüğü korunarak uygulanmıstır.
