Evaluation of Utricular Function in Adults with Bucket Test

ABSTRACT
Objektive: Subjective visual vertical (SVV) defining perception of vertical position is one of the methods used to evaluate utricular function. The aim of this study is to examine the results of the bucket test, which is one of the SVV test methods, in the healthy Turkish young adults. Material and Methods: For this purpose, binocular SVV was evaluated with bucket test in 62 healthy young adults between 18-25 years of age with normal hearing and without vestibular problems. During the bucket test application, the bucket with a mark on its base was placed with the patient’s face in the vertical position, and the bucket was rotated to the level that the patient would see the shape vertical. The application was completed by doing a total of 10 repetitions; 5 clockwise and 5 counterclockwise. The amount of deviation from the vertical was determined in degrees. Results: In the test with the highest absolute values of the vertical deviation, in the analysis of the absolute values of the deviations from the vertical, the median value was 2° (25-75% quartiles= 1-3°) in women and 2.5° (25-75 quartiles= 1-3°) in men. There was no statistically significant difference between female-male scores (p=0.6) and clockwise-counterclockwise rotation scores (p=0.9). Conclusion: In this study, normative results of the bucket test in healthy Turkish young adults are presented. The bucket test, which is an easy-to-use and low-cost SVV measurement method, is recommended for daily practice in vestibular system evaluations.
ÖZET
Amaç: Dikey konum algısını ifade etmek için kullanılan öznel görsel dikey [subjective visual vertical (SVV)] utriküler fonksiyonu değerlendirmek için kullanılan yöntemlerden biridir. Bu çalışmanın amacı, sağlıklı Türk genç popülasyonunda SVV test yöntemlerinden biri olan “kova testinin” (bucket test) sonuçlarını incelemektir. Gereç ve Yöntemler: Bu amaç doğrultusunda yaş aralığı 18-25 yıl olan, işitmesi normal ve vestibüler problemi olmayan 62 sağlıklı genç erişkinde binoküler SVV, kova testi ile değerlendirilmiştir. Kova testi uygulaması sırasında, tabanına işaret yerleştirilmiş kova hastanın yüzü dikey pozisyonda kovanın içine gelecek şekilde yerleştirilmiş ve hastanın şekli dikey gördüğünü belirteceği seviyeye kadar kova döndürülmüştür. Uygulama saat yönünde 5 ve saat yönünün tersine 5 olmak üzere toplam 10 tekrar yapılarak tamamlanmıştır. Dikeyden sapma miktarları derece cinsinden belirlenmiştir. Bulgular: Dikey sapmanın en yüksek mutlak değerlerinin 4° olduğu testte, dikeyden sapmalarının mutlak değerleri analizinde, ortanca değer kadınlar için 2° (%25-75 çeyreklikler= 1-3°) ve erkekler için 2,5° (%25-75 çeyreklikler 1-3°) idi. Kadın-erkek ölçüm değerleri arasında (p=0,6) ve saat yönü-saat yönünün tersi rotasyon ölçüm değerleri arasında istatistiksel açıdan anlamlı bir fark yoktu (p=0,9). Sonuç: Bu çalışmada, SVV değerlendirmesinde kullanılan kova testinin sağlıklı Türk genç erişkinlerdeki normatif sonuçları sunulmuştur. Kullanımı kolay ve düşük maliyetli bir araç olan kova testinin vestibüler sistem değerlendirmesinde kullanımı önerilmektedir.
