Hematological Markers in Idiopathic Chronic Otitis Media with Effusion in Adults

ABSTRACT
Objective: It is not known whether hematological inflammatory
markers, which have been used in the follow-up of many
diseases in recent years, are affected in otitis media with effusion
(OME). The aim of this study was to determine the role of the inflammatory
event in the etiopathogenesis of the OME in adult with hematological
inflammatory markers such as neutrophil-lymphocyte ratio
(NLR), eosinophil-lymphocyte ratio (ELO) and basophil-lymphocyte
ratio (BLR), and the clinical significance of these markers. Material
and Methods: In this study, patients diagnosed with chronic OME and
applied ventilation tube insertion were retrospectively screened. Twenty
five patients between the ages of 18-65 who were diagnosed with unilateral
or bilateral chronic OME without any other health problems constituted
the study group. The control group was consisted of 25 healthy
and drug-free individuals aged 18-65 years. Hematological, demographic
and audiological datas were obtained from medical records.
Results: The mean NLR values of the patients in the study and control
groups were respectively 2.14±1.08 and 1.52±0.42, mean ELR values
were 0.077±0.041 and 0.062±0.040, and mean BLR values were
0.041±0.024 and 0.030±0.012 (p=0.004, p=0.188 and p=0.152, respectively).
The mean NLR values of the of the patients in the reccuring
and non-reccuring groups were respectively 2.00±0.65 and
2.22±1.24, mean ELR values were 0.062±0.043 and 0.084±0.039, and
mean BLR values were 0.031±0.009 and 0.045±0.028 (p=0.137,
p=0.950 and p=0.175, respectively). Conclusion: The significantly
high NLR indicates that an inflammatory event occurs in the etiopathogenesis
of idiopathic chronic OME disease in adults.

ÖZET
Amaç: Son yıllarda birçok hastalığın takibinde kullanılan hematolojik
inflamatuar belirteçlerin efüzyonlu otitis media (EOM) hastalığında
etkilenip etkilenmediği bilinmemektedir. Bu çalışmanın
amacı, erişkinlerde EOM hastalığında nötrofil-lenfosit oranı (NLO),
eozinofil-lenfosit oranı (ELO) ve bazofil-lenfosit oranı (BLO) gibi hematolojik
inflamatuar belirteçler ile hastalığın etiyopatogenezinde inflamatuar
olayın rolünü araştırmak ve bu belirteçlerin klinik önemini
belirlemektir. Gereç ve Yöntemler: Bu çalışmada, kronik EOM tanısı
alıp ventilasyon tüpü takılan hastalar retrospektif olarak tarandı ve 18-
65 yaş aralığında tek taraflı veya çift taraflı kronik EOM tanısı alan
başka bir sağlık sorunu olmayan 25 hasta çalışma grubunu oluşturdu.
Kontrol grubunu ise 18-65 yaşlarında tamamen sağlıklı ve ilaç kullanmayan
25 birey oluşturdu. Hematolojik, demografik ve odyolojik veriler
hasta dosyaları ve hastane bilgi yönetim sisteminden temin edildi.
Bulgular: Çalışma ve kontrol grubunda hastaların sırasıyla ortalama
NLO değerleri 2,14±1,08 ve 1,52±0,42, ortalama ELO değerleri
0,077±0,041 ve 0,062±0,040, ortalama BLO değerleri ise 0,041±0,024
ve 0,030±0,012 olarak bulundu (sırasıyla p=0,004, p=0,188 ve
p=0,152). Nüks eden ve etmeyen hasta grubunda sırasıyla ortalama
NLO değerleri 2,00±0,65 ve 2,22±1,24, ortalama ELO değerleri
0,062±0,043 ve 0,084±0,039, ortalama BLO değerleri ise 0,031±0,009
ve 0,045±0,028 olarak bulundu (sırasıyla p=0,137, p=0,950 ve
p=0,175). Sonuç: NLO’nun anlamlı olarak yüksek bulunması, erişkinlerde
idiyopatik kronik EOM hastalığının etiyopatogenezinde inflamatuar
bir olayın gerçekleştiğini göstermektedir.
