Investigation of the Effect of Selenium in the Treatment of Idiopathic Sudden Sensorineural Hearing Loss

ABSTRACT
Objective: To investigate the effectiveness of selenium,
an antioxidant, in the treatment of idiopathic sudden sensorineural hearing
loss (ISSNHL). Material and Methods: Patients between the ages
of 18-65 who met the inclusion criteria and were treated with systemic
steroid or systemic steroid with selenium due to ISSNHL were included
in the study. The patients were divided into two groups according to
the treatment. Patients using systemic steroid therapy were included in
Group 1, and selenium added to systemic steroid therapy were included
in Group 2. The two groups were compared using hearing gain, recovery
rate and Furuhashi criteria to evaluate treatment efficiency. Results:
Group 1 consisted of 40 patients and Group 2 consisted of 32 patients.
The two groups were homogeneous according to audiological and demographic
data and prognostic factors. When the two groups were compared
in terms of the benefit they received from the treatment by using
hearing gains, recovery rate and Furuhashi criteria, no statistically significant
difference was found between the two groups in all three parameters.
(p=0.355, p=0.154, p=0.754 respectively). Conclusion:
Selenium added to steroid did not reveal a statistically significant difference
in terms of improvement in ISSNHL treatment. Therefore, we
think that selenium, which is thought to be theoretically beneficial in
ISSNHL because it is an antioxidant, but lacks sufficient evidence about
its role in treatment, is not necessary in the treatment of ISSNHL in
order to avoid possible drug side effects and polypharmacy.

ÖZET
Amaç: Bu çalışmanın amacı, ani işitme kaybı (AİK) tedavisinde
bir antioksidan olan selenyumun etkinliğini araştırmaktır. Gereç
ve Yöntemler: Çalışmaya dâhil edilme kriterlerini sağlayan 18-65 yaş
aralığında, AİK nedeni ile sistemik steroid ya da sistemik steroid ile
birlikte selenyum kullanılarak tedavi edilen hastalar dâhil edildi. Hastalar
aldıkları tedaviye göre iki gruba ayrıldı. Grup 1’e sistemik steroid
tedavisi kullanan hastalar, Grup 2’ye ise sistemik steroid ile birlikte selenyum
tedavisi kullanan hastalar dâhil edildi. İki grup tedavi etkinliği
açısından işitme kazancı, işitme geri dönüş yüzdesi ve Furuhashi kriterleri
kullanılarak karşılaştırıldı. Bulgular: Grup 1, 40 hastadan; Grup
2 ise 32 hastadan oluşmaktaydı. İki grup odyolojik ve demografik veriler
ile prognostik faktörler açısından homojendi. Gruplar, tedaviden
gördükleri yarar açısından işitme kazançları, işitme geri dönüş yüzdeleri
ve Furuhashi kriterleri kullanılarak karşılaştırıldığında, iki grup arasında
her üç parametrede de istatistiksel olarak anlamlı fark
bulunmamıştır. (sırasıyla p=0,355; p= 0,154; p=0,754). Sonuç: AİK tedavisinde,
steroide eklenen selenyum, iyileşme açısından istatistiksel
olarak anlamlı bir fark ortaya koymamıştır. Bir antioksidan olduğu için
AİK’de teorik olarak faydalı olabileceği düşünülen, ancak tedavideki
yeri hakkında yeterli kanıt olmayan selenyumun, olası ilaç yan etkilerinden
ve polifarmasiden kaçınmak amacıyla AİK tedavisinde kullanımının
gerekli olmadığını düşünmekteyiz.
