ABSTRACT
The aim of the study was to evaluate long term outcomes after endoscopic sinus surgery (ESS) in the patients with nasal polyposis, chronic sinusitis and rhinogenic headache. 667 patients were enrolled in this study whom were operated in 1995 to 2005. The evaluation of patients were performed in th- ree groups as: chronic sinusitis, nasal polyps and rhinogenic headache. The preoperative and postoperative diagnostic findings of nasal endoscopy, paranasal sinus computed tomograpgy (BT) and skin prick test were examined retrospectively. ESS was performed on patients under controlled hypotension local anesthesia by using the Messerklinger technique. We usually performed infundibulectomy, maxillary ostium dilatation and anterior ethmoidectomy for patients. A significant decreasing in the symptoms of nasal polyps and chronic sinusitis and rhinogenic headache (p <0,05) was found in all patients. 89 patients underwent revision ESS (13.34%). ESS was associated with a less than 1% major complication rate and was successful at least in 80% of patients. ESS is an effective treatment way and can obtain significant symptomatic benefits for patients
ÖZET
Bu çalısmanın amacı; endoskopik sinüs cerrahisinin (ESC) nazal polipozis, kronik sinüzit ve rinojenik bas ağrılı hastalardaki uzun dönem sonuçlarının karsılastırılmasıdır. 1995–2005 yılları arasında operasyonu yapılan 667 hasta çalısmaya dahil edildi. Değerlendirme kronik sinüzit, nazal polip, rinojenik bas ağrılı olgular olmak üzere 3 ayrı grupta yapıldı. Hastaların preoperatif ve postoperatif mevcut diagnostik endoskopi, paranazal sinüs bilgisayarlı tomoğrafisi ve deri prick testi sonuçları retrospektif olarak incelendi. ESC, kontrollü hipotansiyon ile lokal anestezi altında Messerklinger tekniği ile uygulandı. Hastalara en sık infundibulotomi ile birlikte maksiller ostium genisletilmesi ve anterior etmoidektomi yapıldı. Postoperatif dönemde; rinojenik bas ağrısı, kronik sinüzit ve nazal poliplerde anlamlı iyilesmeler gözlendi (p <0,05). 89 olguya revizyon ESC uygulandı (%13.34). ESC’de, %1’den az major komplikasyonlar ve en az %80 oranında basarılı sonuçlar tespit edildi. ESC, kronik rinosinüzit, nazal polipli ve rinojenik basağrısı olan hastalarda güvenli ve etkili uygulanabilir bir cerrahi prosedürdür. Yetersiz cerrahi müdahale revizyon gerekliliğini kaçınılmaz kılar.
