Cholesteatoma which can cause various complications range from mild hearing loss to intracranial infections has a propensity for growth, bone destruction and chronic infection, and should be treated surgically as soon as possible because of the probability to progress very destructively. In this report, two cases of cholesteatoma extending to cranial fossas and going with complication are reported. Posterior cranial fossa extension with labyrinthine invasion and perisinusal abscess development were observed in the 35-year-old first patient who applied with the meningitis, whereas posterior and middle cranial fossa extensions were observed in the 26-year-old second patient. Our aim in this article is to discuss the difficulties and the current improvements in the diagnosis and treatment of chronic otitis media with cholesteatoma extending to cranial fossae.

Hafif işitme kaybından intrakraniyal infeksiyonlara kadar değişen komplikasyonlara yol açabilen kolesteatom; büyümeye, kemik erozyonuna ve kronik enfeksiyona eğilimlidir ve ciddi destrüktif seyredebilmesi sebebiyle olabildiğince erken cerrahi olarak tedavi edilmelidir. Bu raporda kraniyal fossalara uzanımı olan ve komplikasyonla seyreden kolesteatomlu kronik otitis medialı iki vaka bildirilmiştir. Otuzbeş yaşında menenjit kliniği ile başvuran ilk hastada arka kafa çukuruna uzanımla birlikte labirentin invazyon ve perisinüzal apse gelişimi izlenirken, yirmialtı yaşında ikinci hastada arka ve orta kafa çukuruna uzanım izlenmiştir. Bu yazıda amacımız kraniyal fossa uzanımı olan kolesteatomlu kronik otitis mediaların tanı ve tedavisinde karşılaşılan zorlukları ve güncel gelişmeleri tartışmaktır.
