ABSTRACT
Objective: Benign vocal fold pathologies are the most
quality of voice deteriorating etiologic factor after laryngeal infectious
and inflammatory diseases considering quality of voice. The most common
organic pathologies requiring surgical treatment are vocal cord
polyps and reinke edema. These lesions change the frequency and intensity
of the voice, besides may cause the sound breaks. Purpose of
the present study is to evaluate the effectiveness of cold knife technique
with surgery of vocal fold polys and reinke edema. Material and
Methods: Fifty-three (22 female, 31 male) adult patients with vocal
cord polyp or reinke edema were included in the study. Acoustic voice
analysis was performed with Multi Dimentional Voice Program in all
patients preoperatively and in the first postoperative month. Results:
Postoperative, jitter percentage (Jitt), shimmer percentage (Shim) and
noise-harmonic ratio measurements were significantly lower compared
to preoperative values. The fundamental frequency (Fo) values also increased
statistically in the postoperative period within females operated
for reinke edema. Conclucion: The results support that, cold knife
technique in endolaryngeal microsurgery can applied successfully to
organic lesions.

ÖZET
Amaç: Ses kalitesini, enfeksiyöz ve inflamatuar hastalıklardan
sonra en çok bozan etiyolojik nedenler benign vokal kord patolojileridir.
En sık cerrahi tedavi gerektiren organik patolojiler vokal kord
polipleri ve reinke ödemidir. Bu lezyonlar sesin frekansını ve şiddetini
değiştirip, ayrıca seste kırılmalara yol açar. Çalışmanın amacı,
vokal kord polibi ve reinke ödemi patolojilerinde soğuk bıçak tekniği
ile uygulanan cerrahi tedavinin etkinliğini objektif olarak değerlendirmektir.
Gereç ve Yöntemler: Çalışmaya 53 (22 kadın, 31 erkek)
vokal kord polip veya reinke ödemi olan erişkin hasta dâhil edilmiştir.
Tüm hastalara preoperatif ve postoperatif 1. ayda “Multi Dimentional
Voice Program” ile akustik ses analizi yapılmıştır. Bulgular:
Tüm hastalar incelendiğinde, postoperatif jitter yüzdesi (Jitt), Shimmer
yüzdesi (Shim) ve gürültü-harmonik oranı ölçümleri preoperatif
döneme göre istatistiki olarak belirgin düzeyde azalmıştır. Temel frekans
(Fo) değerleri ise reinke ödemi olan kadın hastalarda postoperatif
dönemde istatistiksel olarak artmıştır. Sonuç: Elde edilen
sonuçlar, endolaringeal mikrocerrahide soğuk bıçak tekniğinin, organik
patolojilerde başarıyla uygulanabilir bir teknik olduğunu desteklemektedir.
