Otitis Media Associated with ANCA Positive Vasculitis as a Cause of Recurren Failure Surgery

ABSTRACT
Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) are chronic, autoimmune diseases characterized by inflammation of small vessels. A 30-year-old woman who had a history of 5 right ventilation tubes, tympanoplasty, eustachian tube prosthesis, and septoplasty presented to our clinic with the complaint of right hearing loss for 6 years. Physical examination revealed sequelae changes in the right tympanum, perforation of the nasal septum and intense black nasal crust. Odiometric examination revealed a 48 dB conductive type hearing loss in the right ear. Complement, MPO-ANCA and Angiotensin-converting enzyme were normal. PR3-ANCA was positive. Steroid + immunosuppressive combined therapy was applied. Otitis media caused by ANCA-positive associated vasculitis (OMAAV) is a rare condition. To avoid recurrent unsuccessful surgery, OMAAV should be considered in differential diagnosis in resistant otitis media.
ÖZET
Antinötrofil stoplazmik antikor (ANCA) ilişkili vaskülitler (AİV) küçük damarların enflamasyonu ile karakterize, süregen, otoimmün hastalıklardır. Özgeçmişinde 5 sağ ventilasyon tüpü, timpanoplasti, östaki tüp protezi ve septoplasti hikayesi olan 30 yaşında kadın olgu, kliniğimize 6 yıldır var olan sağ işitme kaybı şikayeti ile başvurdu. Fizik muayenede sağ timpan membranda sekel değişiklikler, nazal septum perforasyonu ve yoğun siyah nazal krut mevcuttu. Odiometrik incelemede sağ kulakta 48 dB iletim tipi işitme kaybı mevcuttu. Kompleman, MPO-ANCA ve Anjiotensin konverting enzim normal saptandı. PR3-ANCA pozitifti. Steroid + immünsupresif kombine tedavi uygulandı. ANCA pozitif vaskülitle ilişkili otitis media (AİVOM) nadir bir durumdur. Tekrarlayan başarısız cerrahiden kaçınmak için, tedaviye dirençli otitis medialarda AİVOM ayırıcı tanıda düşünülmelidir.
