Papillary Carcinoma in a Branchial Cleft Cyst: Diagnostic Dilemma

ABSTRACT
Although branchial cleft cysts (BCC) are common, primary cancers are rare. Generally
squamous cell carcinoma is seen in BCC. The diagnosis of branchial carcinoma is established by excision
of the lesion and then demonstrating the sinus-tract relationship on histopathological examination
of the mass. Diagnosis is supported by immunohistochemical studies. In this article, we
reported a 56-year-old male patient, which has admitted to our clinic with swelling on the left side
of the neck for a long time that we operated with the diagnosis of the branchial cleft cyst, but the
histopathology reported as papillary carcinoma.

ÖZET
Brankial kleft kistleri (BKK) sık görülmesine rağmen, primer kanserleri nadir olarak karşımıza
çıkabilmektedir. Genel olarak skuamöz hücreli karsinomlar görülmektedir. Brankial karsinomların
tanısı, lezyonun çıkarılması ve ardından kitlenin histolojik incelemesinde sinüs-trakt
ilişkisinin ispatı ile konulmaktadır. İmmünohistokimyasal çalışmalar ile tanı desteklenebilmektedir.
Bu çalışmada, boynunun solunda uzun zamandır var olan şişlik şikâyetiyle kliniğimize başvuran,
brankiyal kleft kisti ön tanısı ile opere ettiğimiz, ancak patolojisi papiller karsinom olarak rapor
edilen 56 yaşındaki erkek olgunun sunulması amaçlanmıştır.
