Primary Sjögren's syndrome is an autoimmune disease that is characterized by the attacks and leads to the hypofunction of the exocrine glands. It most commonly affects salivary and lacrimal glands. In addition, it may affect the other organ systems, such as musculoskeletal system, lung, kidney, liver, reticuloendothelial system and skin. Initially, the disease usually begins with very mild bilateral parotid swelling. Rarely joint symptoms such as arthralgia or morning stiffness might be prominent in attacks. Over time, the severity of parotid swelling increases with recurrent attacks. Dryness of mouth and eye and the complaint of pain becomes apparent, which may sometimes last for years. The initial attack may rarely be severe at the primary Sjögren's syndrome. The disease may resemble the setting of the acute parotitis with the severe parotid swelling, pain and even fever. In this article, it was emphasized that there should be high index of suspicion about autoimmune diseases in patients with parotid swelling at the acute and bilateral phase and Sjogren's syndrome should be considered primarily.

Primer Sjögren sendromu ataklar ile seyreden, ekzokrin bezlerin hipofonksiyonuna neden olan bir otoimmün hastalıktır. En sık tükürük ve gözyaşı bezlerini etkiler. Bunun yanı sıra kas-iskelet sistemi, akciğer böbrek, karaciğer, retiküloendotelyal sistem ve deri gibi organları da tutabilir. Hastalık genellikle başlangıçta çok hafif bilateral parotis şişliği ile başlar. Nadiren ataklarda artralji, sabah tutukluğu gibi eklem bulguları öne çıkabilir. Zaman içerisinde tekrarlayan ataklarda parotiste şişliğin şiddeti artar. Ağız ve göz kuruluğu gibi ağrı şikayeti belirginleşir. Bu durum bazen yıllar alabilir. Primer Sjögren sendromunda nadiren başlangıç atağı şiddetli olabilir. Hastalık şiddetli parotis şişliği, ağrı, hatta ateş ile akut parotit tablosuna benzeyebilir. Bu makalede bilateral ve akut başlangıçlı parotis şişliği olan hastalarda otoimmün hastalıktan şüphelenilmesi ve öncelikli olarak Sjögren sendromunun düşünülmesi gerekliliği vurgulandı.
