The neck was reconstructed in 15 patients, who developed large skin and mucosa defects due to pharyngocutaneous fıstula after total laryngectomy and neck dissection for laryngeal squamous cell carcinoma. The neck skin was reconstructed with a deltopectoral flap and anterior pharyngeal wall was repaired with a pectoralis major musculocutaneous flap, both in one stage. This study describes effects of infection and necrosis in long-standing fıstula, creation of a controlled pharyngostoma, timing for surgical repair of fistula, design and utilization of deltopectoral and pectoralis major flaps, and delay procedures of deltopectoral flap for neck reconstruction.

Larenks yassı hücreli kanseri tanısıyla total larenjektomi ve boyun diseksiyonu yapılan ve posto-peratif dönemde farengo-kutanöz fistüle bağlı olarak geniş boyun cildi ve farenks mukoza defekti oluşan 15 hastada, boyun cildi deltopektoral flep ile ve farenks ön duvarı da pektoralis majör kaslı-cilt flebi ile aynı seansta onarıldı. Bu hasta serisinde, fıstüllere bağlı derin boyun enfeksiyonu ve nekroz., farengostoma oluşturulması, fıstül tamirinde zamanlama, deltopektoral ve pektoralis majör fleplerinin tasarımı ve cerrahi teknik, diğer tamir yöntemlerine göre farklar ve deltopektoral flebin geciktirilmesi ve boyun rekonstrüksiyonundaki kullanımı tartışıldı.
