Revision Endoscopic Sinus Surgery: Surgical Outcomes

ABSTRACT
To study common reasons and surgical efficacy of our experience in revision endoscopic sinus surgery (ESS) in adult patients with chronic rhinosinusitis and recurrent nasal polyps. Material and Methods: Revision ESS is indicated for patients who remain symptomatic after primary ESS and optimal medical therapy. A retrospective study was conducted on 89 adult patients who underwent revision ESS, selected from 667 consecutive primary ESS patients in our department between 1995 and 2005. We performed revision ESS on chronic sinusitis and polyps under controlled hypotensive anesthesia using the Messerklinger technique. Results: The average follow-up was 18 months. There were 14 cases of recurrence of polyps (15.73%) and 75 cases of recurrence of sinusitis (84.26%). The most common anatomic factor associated with primary surgical failure was lateralization of the middle turbinate (78%) followed by incomplete anterior ethmoidectomy (64%), scarred frontal recess (50%), incomplete posterior ethmoidectomy (41%), and middle meatal antrostomy stenosis (39%). Revision ESS was associated with a 4.49% major complication rate and was successful in 84.26% of patients. Conclusion: Revision ESS is most often associated with anatomic obstruction in the area of the ostiomeatal complex and the inexperienced surgeon. Revision ESS is an effective way of treatment and the patient can obtain significant symptomatic benefit for chronic rhinosinusitis.
ÖZET
Bu çalışmada kronik rinosinüzitli ve rekürren nazal polipli erişkin hastalarda, revizyon endoskopik sinüs cerrahisi (ESC)’nin sık görülen nedenleri, etkinliği ve klinik sonuçlarımız değerlendirildi. Yöntem ve Gereçler: Revizyon ESC, primer ESC ve uygun medikal tedaviye rağmen semptomları devam eden hastalara uygulandı. 1995 ve 2005 yılları arasında, kliniğimizde primer ESC uygulanan 667 hasta içerisindeki 89 erişkin revizyon ESC hastası retrospektif olarak incelendi. Kronik rinosinüzitli ve nazal polipli hastalara kontrollü hipotansif anestezi altında Messerklinger tekniği ile revizyon ESC operasyonu yapıldı.. Bulgular: Ortalama takip süresi 18 ay idi. Rekürrens tespit edilen hastaların 14’ünde nazal polip (%15.73), 75’inde ise (%84.26) kronik sinüzit tespit edildi. Primer cerrahi sonrası başarısızlığın en önemli nedenleri; orta konka lateralizasyonu (%78) ve bunu takiben yetersiz ön etmoidektomi (%64), frontal reses skatrizasyonu (%50), yetersiz arka etmoidektomi (%41) ve orta meatal antrostomi stenozu (%39) olarak sıralandı. Revizyon ESC’de komplikasyon oranı %4.49 olarak bulundu ve hastaların %84.26’sında başarılı sonuçlar alındı. Sonuç: Revizyon ESC, sıklıkla ostiomeatal kompleks bölgesinde anatomik obstrüksiyonlar ve cerrahın deneyiminin az olması ile ilişki-lidir. Kronik sinüzit tedavisinde revizyon ESC, belirgin semptomatik rahatlama sağlayan, etkili bir tedavi yöntemidir.
