The Effect of Septoplasty and Endoscopic Sinus Surgery on Voice Aerodynamics and Voice-Related Quality of Life

ABSTRACT
Objective: To determine the aerodynamic change in
voice, its effect on voice-related quality of life in patients who underwent
endoscopic sinus surgery (ESS) and/or septoplasty. Material and
Methods: Demographic characteristics, medical history, complaints,
operation were prospectively recorded. Voice evaluations were performed
preoperatively and at 3 months postoperatively. Subjective and
objective voice assessment were done with the Voice-Related Quality
of Life (VRQoL) Scale and Voice Handicap Index, maximum phonation
time (MPT) and S/Z ratio. Results: Our study was conducted with
a total of 42 patients, 20 (47.6%) male and 22 (52.40%) female. The
mean age was 39.02±14.38 years. Septoplasty were performed to 24
(57.14%) patients, and septoplasty and ESS were performed to 18
(42.86%) patients. While 23 of the patients (54.8%) had a history of
intubation due to another surgery, 28 (66.7%) of them stated that they
experienced preoperative voice changes.. However, no statistically significant
difference was found between the complaint of preoperative
voice change and intubation history or gender (p>0.05). It was observed
that the MPT was prolonged after surgery (p=0.003), particularly in
men (p=0.005). The social, physical and total VRQoL increased after
surgery (p=0.035, p=0.002, p=0.001, respectively). There was a greater
improvement in total VRQoL Scale values in patients who underwent
ESS (23.83 vs. 2.62; p=0.005). Conclusion: Our study shows that septoplasty
and ESS may increase voice-related quality of life and have a
positive effect on the aerodynamic structure of voice. More studies are
needed to confirm our results.

ÖZET
Amaç: Endoskopik sinüs cerrahisi (ESC) ve/veya septoplasti
yapılan hastalarda, sesteki aerodinamik değişimin ve sese bağlı yaşam
kalitesine etkisinin değerlendirilmesidir. Gereç ve Yöntemler: Çalışmamız
prospektif olup, hastaların demografik bilgileri, medikal geçmişi,
şikâyetleri, yapılan operasyon kaydedildi. Ses değerlendirmeleri
preoperatif ve postoperatif 3. ayda yapıldı. Subjektif ses değerlendirmesi
Sesle İlgili Yaşam Kalitesi Ölçeği (SİYKÖ) ve Ses Handikap Endeksi
ile yapıldı. Objektif ses değerlendirmesi, aerodinamik
parametreler olan maksimum fonasyon zamanı (MFZ) ve S/Z oranı değerlendirilerek
yapıldı. Bulgular: Çalışmamız 20’si (%47,6) erkek,
22’si (%52,40) kadın olmak üzere toplam 42 hastayla yapıldı. Hastaların
yaş ortalaması 39,02±14,38 idi. Hastaların 24’üne (%57,14) septoplasti,
18’ine (%42,86) ise septoplasti ve ESC yapıldı. Hastaların
23’ünde (%54,8) başka bir cerrahi nedeniyle entübasyon öyküsü mevcutken,
28’i (%66,7) preoperatif ses değişiklikleri yaşadığını ifade etti.
Ancak preoperatif ses değişikliği şikâyeti ile entübasyon öyküsü ya da
cinsiyet arasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05).
Cerrahi sonrasında öncesine göre MFZ’nin uzadığını (p=0,003) ve erkeklerde
MFZ’deki değişimin istatistiksel olarak daha anlamlı olduğunu
(p=0,005), ayrıca sosyal, fiziksel ve total sesle ilişkili yaşam
kalitesinin de arttığı görüldü (sırasıyla p=0,035, p=0,002, p=0,001).
ESC yapılanlarda total SİYKÖ değerlerinde daha fazla düzelme olduğu
saptandı (23,83’e karşı 2,62; p=0,005). Sonuç: Çalışmamız, ESC ve
septoplastinin sesle ilişkili yaşam kalitesini artırdığını, sesin aerodinamik
yapısı üzerine pozitif etkisinin olduğunu göstermektedir. Sonuçlarımızın
teyidi için daha çok çalışmaya ihtiyaç vardır.
