The Efficacy of Uvulopalatal Flap Surgery in Patients With Obstructive Sleep Ap-nea
Obstrüktif Uyku Apne Sendromlu Hastalarda Uvulopalatal Flep Cerrahisinin Etkinliği

ABSTRACT
Objective: To evaluate the subjective and objective outcomes of uvulopalatal flap surgery in patients with obstructive sleep apnea syndrome (OSAS). Material and Methods: Uvulopalatal flap operation was applied to 37 patients with OSAS under general anesthesia. All patients’ medical history, body mass index and neck circumference measurements were recorded before surgery. A fiber optic endoscopy with Muller’s maneuver was performed to evaluate the level of obstruction in the upper airway. Daytime sleepiness was estimated by the Epworth sleepiness scale. All patients were evaluated before surgery by nocturnal polysomnography and variables of polysomnography included sleep stages, sleep efficiency, apnea-hypopnea index, mean oxygen saturation, minimal oxygen saturation, oxygen desaturation index were recorded. After the surgery all patients were reevaluated with Epworth sleepiness scale and nocturnal polysomnography were reperformed. Surgical success was defined as achieving the postoperative apnea-hypopnea index to less than 20 and %50 or greater reduction in postoperative apnea-hypopnea index as compared with preoparative value. Results: Our surgical success rate showed %54.05 after the uvulopalatal flap surgery. However; apnea-hypopnea index and oxygen desaturation index showed a statistically significant decrease after uvulopalatal flap surgery (p<0.05). Minimal oxygen saturation showed a statistically significant increase in the postoperative period (p<0.05) but mean oxygen saturation showed no statistically significant increase (p>0.05). Epworth sleepiness scale scores showed a statistically significant decrease after surgery (p<0.05). Al-though no statistically significant changes were observed in sleep stages after surgery (p>0.05), sleep efficiency showed a statistically significant increase with surgery ( p<0.05). Conclusion: The results of this study highlights that uvulopalatal flap surgery improves obstructive sleep apnea symptoms with minimal adverse effect in selected patients.
ÖZET
Amaç: Obstrüktif uyku apne sendromlu (OUAS) hastalarda uvulopalatal flep operasyonunun etkinliğinin objektif ve subjektif olarak değerlendirilmesi. Yöntem ve Gereçler: OUAS’lu 37 hastaya genel anestezi altında uvulopalatal flep operasyonu uygulandı. Bütün hastaların operasyon öncesi anamnezleri alınarak, vücut kitle indeksi ve boyun çevresi ölçümleri kaydedildi. Fiberoptik endoskopi ile Müller manevrası yapılarak üst hava yollarındaki obstrüksiyon seviyesi değerlendirildi. Gün içi uyuklama hali Epworth uykululuk skalası ile değerlendirildi. Bütün hastalara operasyon öncesi gecelik polisomnografi yapılarak hastaların uyku evreleri, uyku etkinliği, apne-hipopne indeksi, ortalama O2 saturasyonu, minimal O2 saturasyonu, O2 desaturasyon indeksi kaydedil-di. Operasyon sonrası bütün hastalar Epworth uykululuk skalası ve gecelik polisomnografi yapılarak yeniden değerlendirildi. Operasyon sonrası apne-hipopne indeksi değerinin 20’nin altına düşmesi ve operasyon öncesi apne-hipopne indeksi değeriyle karşılaştırıldığında operasyon sonrası apne-hipopne indeksi değerinde %50 veya üzerinde azalma olması cerrahi başarı olarak ifade edildi. Bulgular: Uvulopalatal flep operasyonu sonrası cerrahi başarı oranımız %54.05 olarak saptandı. Bununla birlikte, uvulopalatal flep operasyonu sonrası apne-hipopne indeksi ve O2 desaturasyon indeksi değerlerinde istatistiksel olarak anlamlı bir azalma olduğu saptandı (p<0.05). Operasyon sonrası minimal O2 saturasyonunda istatistiksel olarak anlamlı bir artış saptandı (p<0.05) fakat ortalama O2 saturasyonunda istatistiksel olarak anlamlı bir artış saptanmadı (p>0.05). Operasyon sonrası Epworth uykululuk skalası skorlarında istatistiksel olarak anlamlı bir azalma saptandı (p<0.05). Operasyon sonrası uyku evrelerinde istatistiksel olarak anlamlı bir değişiklik gözlenmemesine rağmen (p>0.05), uyku etkinliğinde istatistiksel olarak anlamlı bir artış saptandı (p<0.05). Sonuç: Bu çalışmanın sonuçlarıyla uvulopalatal flep operasyonunun seçilmiş hastalarda çok az komplikasyonla beraber obstrüktif uyku apnesinde semptomları düzelttiği sonucuna ulaşıldı.
