Thirty-three patients who required facial nerve manipulation during cranial base surgery were reviewed in this study. Twenty-two of them underwent only nerve displacement for surgical access to cranial base. Most of the surgical approach for cranial base surgery have preserved facial nerve. Modern imaging techniques and familiarity with the complex anatomy of the seventh nerve facilitate preoperative planning of the most suitable surgical approach. The potential facial paralysis may be anticipated, allowing the patient to be well informed about any possible temporary or permanent facial paralysis and to understand and cooperate with subsequent rehabilitation. Using current techniques of skull base surgery, the facial nerve no longer should present a limitation to safe exposure and adequate removal of skull base lesions.

Bu çalısmada, fasiyal sinire yönelik cerrahi islem yapılan 33 kafa tabanı olgusu sunulmustur. Bunların 22’sinde, yalnız sinir deplasmanı yapılarak cerrahi ulasım sağlanmıstır. Kafa tabanı cerrahisinde yaklasımların çoğu fasiyal siniri korumaktadır. Fasiyal sinirin kompleks anatomisinin bilinmesi ve modern görüntüleme yöntemleri ameliyat öncesi en uygun cerrahi yaklasımın planlamasını kolaylastırmıstır. Hastaya geçici ya da kalıcı muhtemel fasiyal paralizi konusunda bilgi verilmeli ve müteakip rehabilitasyonun uzun sürebileceği söylenmelidir. Kafa tabanı cerrahisinde çağdas teknikleri kullanarak, kafa tabanı lezyonlarına gerekli bir sekilde ekspojur sağlanması ve yeterince eksize edilmesinde artık fasiyal sinir bir sınırlama getirmemelidir.
