Bilateral abduktor vokal kord paralizisi sık karşılaşılan bir klinik tablo değildir. En sık görülen etyolojik neden tiroid cerrahisidir. Tedavisinde pek çok cerrahi metod denenmiş fakat arzulanan hedefe ulaşılamamıştır. Denenmiş ve tanımlanmış birçok cerrahi metodun olması, arzulanan hedefe ulaşmanın zorluğunun bir göstergesidir. Bu çalışmamızda bilateral abduktor paralizi nedeniyle endolarengeal arytenoidektomi yaptığımız 15 hasta takdim edilmiş, gelişen teknoloji ve uygulanan yeni metodların yanında Thornell ameliyatının geçerliliğini koruduğu vurgulanmaya çalışılmıştır.
