The occurence of adverse reactions to allergen spesific immunotherapy is rare. Most of them appear within 30 minutes after injection. The reactions occurring after 30 minutes are called as late side effects. Usually treatment dropout is not required. In this paper, we present a case that required discontinuation of the treatment because of developing of a late systemic side effect that had never been reported before. Subcutaneous pre-seasonal immunotherapy was planned to a 23 year-old man with seasonal allergic rhinitis. The patient received subcutaneous pre-seasonal immunotherapy for 2 times with 1 week interval. Ten hours after first administration, he had complaints of fever, discomfort, myalgia and tiredness. Planned immunotherapy regimen was continued and same complaints were repeated 12 hours after second injection. Fever could not be controlled with medication and went on for 3 days. Complaints of generalized myalgia, discomfort, artralgia and tiredness accompanied with the fever. These clinical findings were suggested as systemic adverse reactions of immunotherapy and the treatment is discontinued. Patients should be informed about the side effects that could appear in late period and in case of side effect occuring, the treatment protocol should be overviewed again.

Subkütan alerjen spesifik immünoterapi sonrası sistemik yan etkiler nadir görülür. Bunların büyük bir kısmı enjeksiyonu takip eden 30 dakika içinde görülür. 30 dakikadan sonra görülenler geç yan etkiler olarak kabul edilir. Yan etkiler sonrası tedavinin kesilmesine genellikle ihtiyaç olmaz. Bu yazıda, immünoterapi sonrası geç sistemik yan etki gelişen ve tedavinin kesilmesini gerektiren bir olgu sunulmuştur. 23 yaşında erkek hastaya, mevsimsel alerjik rinit şikayetleri ile mevsim öncesi immunoterapi planlandı. Subkütan mevsim öncesi immünoterapi bir hafta ara ile 2 kez uygulandı. Hastaya ilk yapılan enjeksiyondan 10 saat sonra başlayan ateş, halsizlik, miyalji ve yorgunluk şikayetleri oldu. Planlanan tedavi protokolü uygulanmaya devam edildi. İkinci enjeksiyonu takiben 12 saat sonra aynı şikayetler tekrar gözlendi. Ateş antipiretiklerle kontrol altına alınamadı ve 3 gün devam etti. Bu bulgular, immünoterapinin sistemik yan etkisi olarak kabul edildi ve tedavi sonlandırıldı. Hastalar geç dönemde görülebilecek yan etkiler açısından bilgilendirilmeli, yan etki gelişmesi durumunda tedavi planı gözden geçirilmelidir.
