Transient Global Amnesia After Positional Testing and Canalolith Repositioning Maneuver in Benign Paroxysmal Positional Vertigo

ABSTRACT
Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo. It forms an significant number of patients in ear nose throat and neurology clinics. Transient global amnesia (TGA) is a rare neurological entity characterized by sudden onset anterograde amnesia and sometimes retrograde amnesia, which resolves within 24 hours at the latest. It is particularly triggered by emotional and physical stress. Its differential diagnosis includes transient ischemic attack in the posterior cerebral circulation, focal seizures, postictal state, psychogenic amnesia, post-traumatic amnesia, and metabolic disorders such as hypoglycemia. In this case, a 72-year-old female patient who experienced sudden changes in consciousness and loss of orientation shortly after performing the Dix-Hallpike test and then the Epley maneuver due to posterior canal BPPV is presented. As a result of the examinations, it was evaluated as TGA and returned to normal within 24 hours.
ÖZET
Benign paroksismal pozisyonel vertigo (BPPV) en yaygın periferik vertigo nedenidir. Kulak burun boğaz ve nöroloji kliniklerinin önemli bir hasta bölümünü oluşturur. Geçici global amnezi (GGA), ani başlangıçlı anterograd amnezi ve bazen de retrograd amnezi ile karakterize, en geç 24 saat içinde düzelen nadir bir nörolojik antitedir. Özellikle duygusal ve fiziksel stres tarafından tetiklenir. Ayırıcı tanısı posterior serebral dolaşımda geçici iskemik atak, fokal nöbetler, postiktal durum, psikojenik amnezi, travma sonrası amnezi ve hipoglisemi gibi metabolik bozuklukları içerir. Bu olguda, posterior kanal BPPV nedeniyle önce Dix-Hallpike testi ardından Epley manevrası uygulandıktan kısa süre sonra ani bilinç değişikliği ve oryantasyon kaybı yaşayan, yapılan incelemeler sonucunda GGA olarak değerlendirilen ve 24 saat içinde normale dönen 72 yaşında kadın hasta sunulmuştur.
