Microbiological Comparison of Polyvinyl Acetal and Fingerstall Packs Following Endonasal Surgery

ABSTRACT
Objective: To compare the microbiological culture results and bacterial colonization of two different nasal packing materials (fingerstall packs and polyvinyl acetal packs) used in endonasal surgery. Material and Methods: 40 consecutive patients who underwent septoplasty or septorhinoplasty were randomly divided into two groups. In 18 patient who formed the first group polyvinyl acetal nasal packs (Merocel®) were used postoperatively while fingerstall packs were used for 22 patients who take part in the second group. Postoperatively profilactic amoxicillin + potassium clavulanate was given to all patients. After the removal, surface measurement of the nasal packs was performed and they were preserved in sterile pots. For each cm2 1 ml 0.9% NaCl solution was added to sterile pots and microbiological sampling was performed from this solution that contact with whole surface of the nasal packs. The microbiological results of the two groups were compared. Result: The sex and age distribution of the two groups were not significantly different (p=0.577 ve 0.714 respectively). The bacterial colonization rates in 44 fingerstall packs and 36 polyvinyl acetal nasal packs were found 81.8% and 72.2% respectively. The rates of bacterial colonization were not significantly different between the two groups (p= 0.307). In both groups coagulase negative staphylococcus was found to be the most frequent colonized bacteria. Staphylococcus aureus, Staphylococcus epidermidis and Pseudomonas species were the other frequent bacterial agents. Conclusion: Despite we expect lesser bacterial colonization in fingerstall packs due to features of the surface structure, polyvinyl acetal nasal packs were not lead to increased bacterial colonization rates when compared to fingerstall nasal packs.
ÖZET
Amaç: Endonazal cerrahi sırasında kullanılan iki farklı tampon materyalinin (eldiven parmağı tampon ve polivinil asetal tampon) mikrobiyolojik açıdan incelemek ve bakteriyal kolonizasyon oranlarını karşılaştırmak. Yöntem ve Gereçler: Septoplasti ve septorinoplasti ameliyatı uygulanan 40 hasta randomize olarak iki gruba ayrıldı. Birinci grubu oluşturan 18 hastaya postoperatif olarak polivinil asetal (Merocel®) tamponlar kullanılırken, ikinci grupta yer alan 22 hastada eldiven parmağı tamponlar kullanıldı. Postoperatif dönemde tüm hastalara oral amoksisilin + potasyum klavulanat profilaktik olarak verildi. Tampon materyalleri çıkarıldıktan sonra yüzey ölçümleri yapıldı ve steril kaplarda saklandı. Steril kaplara her cm2 için 1 ml %0.9 NaCl solüsyonu eklendi ve mikrobiyolojik örnekleme tamponların tüm yüzeyine temas eden sıvıdan yapıldı. İki grubun mikrobiyolojik sonuçları karşılaştırıldı. Bulgular: İki grup arasında yaş ve cinsiyet dağılımları açısından farklılık yoktu (sırasıyla p=0.577 ve 0.714). Hastalarda kullanılan 44 adet eldiven parmağı ve 36 adet polivinil asetal tamponda bakteriyal kolonizasyon oranları sırası ile %81.8 ve %72.2 olarak bulundu. İki grubun bakteriyal kolonizasyon oranları arasındaki fark istatistiksel olarak anlamlı bulunmadı (p= 0.307). Her iki grupta da koagülaz negatif stafilokok en sık kolonize olan bakteri olarak tespit edildi. Staphylococcus aureus, Staphylococcus epidermidis ve Pseudomonas türleri üreyen diğer sık üreyen bakteriyal ajanlardı. Sonuç: Yüzey özelliklerinden dolayı eldiven parmağı tamponlarda bakteriyel kolonizasyon oranlarını daha az beklememize rağmen polivinil asetal tamponların, eldiven parmağı tamponlara göre daha fazla bakteriyal kolonizasyona yol açmadığı görülmüştür.
