Isolated sphenoid sinus pathologies including sphenoid sinusitis are very rare disorders compared to other paranasal sinuses. Close proximity of sphenoid sinus to vital structures and slender bony structures may allow the infection to disseminate with serious neurological complications. VIth nerve is the most frequently affected cranial nerve in sphenoid sinus diseases including neoplasms. In this paper, we report a very rare case of isolated sphenoid sinusitis in a 65 year old woman who was presented with unilateral complete VIth nerve palsy. The abducens nerve palsy recovered after the treatment that consists of endoscopic sphe- noidotomy and parenteral antibiotic therapy. We discuss the clinical features, diagnosis and the treatment for this rare entity.

İzole sfenoid sinüs patolojileri ve enfeksiyonları diğer paranasal sinüslere oranla oldukça nadirdir. Sfenoid sinüsün vital yapılarla komsuluğundan dolayı enfeksiyonun kemik dokuyu asması ciddi nörolojik komplikasyonlarla sonuçlanabilir. VI. kraniyal sinir genelde neoplazi dahil olmak üzere sfenoid sinüs hastalıklarından en sık etkilenen yapıdır. Bu olgu sunumunda VI. kraniyal sinir paralizisine neden olan izole sfenoid sinüzitli 65 yasındaki bir hasta sunulacaktır. Abdusens paralizisi endoskopik sfenoidotomi ve parenteral antibiyotik tedavisi ile düzelmistir. Bu nadir klinik antitenin tanı ve tedavisi güncel literatür bilgileri esli- ğinde tartısılmıstır.
